In spite of the title of this document --- the report is really just that Apollo 17 was an "essentially fault-free flight" --- this is really more of a nostalgic look back at things that had gone wrong in earlier missions which the author refers to as "stuck PIPA", "1300 restarts", "travelling trunnion", and so forth.
