Boston Public Library (Rare Books Department) copy BENTON 509.C28M formerly owned by Sir Henry Halsall, with one inscription recording his donation of the book to Halsall Church, Lancashire, and another that Willm. Welding borrowed the book from Sir Thomas Haskew, priest. Lower margins of leaves cxxxiv-cxxxv each contain 4 lines of Latin marginalia. Full page ms. entitled Oratio on ending flyleaf. With the armorial bookplate of Sir Robert Shafto Adair on front pastedown, and an 1846 letter loosely inserted from F.C. Husenbeth who borrowed the book from Adair for research. Along with this is a typewritten description and a bookdealer's catalog description, possibly from Quaritch whose collation note appears on back pastedown. Inserts are housed in accompanying envelope BENTON 509.C28M Addenda.

Early thumb indexes. Covers coated with shellac. Title leaf and leaves 130 through 137 damaged; first gathering silked. Numbers on endpapers, with extensive early ms inscriptions and corrections on last 2 leaves and flyleaf. Names of popes and Thomas of Canterbury in calendar inked over in compliance with Henry VIII's censorship.
